Uniform Residential Loan Application (Form 1003)

Fannie Mae and Freddie Mac (the GSEs) redesigned the Uniform Residential Loan Application (URLA) (Fannie Mae Form 1003) and created new automated underwriting system (AUS) specifications (Fannie Mae Desktop Underwriter® [DU®] Spec) to help lenders better capture relevant loan application information and support the industry’s move to digitize the loan origination process. The GSEs also created a Uniform Loan Application Dataset (ULAD) to show how Form 1003 is mapped to MISMO® v3.4 Reference Model and a voluntary Supplemental Consumer Information Form (SCIF/Form 1103) to assist lenders in capturing information on borrower homeownership education, housing counseling and language preference information.

Supplemental Consumer Information Form (Form 1103) Updates We’ve issued Lender Letter LL-2022-03 to notify lenders of the mandatory use of the Supplemental Consumer Information Form (SCIF/Form 1103) for loans sold to the GSEs with application dates on or after March 1, 2023. Translated versions of the SCIF are available in Spanish, traditional Chinese, Vietnamese, Korean, and Tagalog on the Multi-Language Resources for Lenders page to help lenders serve borrowers who have limited English proficiency.
